Rights and Responsibilities
Dr. Edwards' patients have the
right to:
- Receive
appropriate, considerate, and respectful treatment
regardless of their race, sex, disability, cultural,
economic, or religious background.
- Be told
about your illness, prognosis, and recommended treatment
in words you understand.
- Be provided
with all the information you need in order to give or
refuse consent for recommended treatment.
- Privacy and
confidentiality of all records and communications about
your care.
- Request a
second opinion from another physician regarding your
diagnosis or proposed treatment.
- Explanation
of charges and discussion of the cost of your treatment
services.
- Refuse
recommended treatment.
- An
explanation of the likely outcome if you refuse
treatment.
- Express
concerns about your care or service to Dr. Edwards.
Dr. Edwards patients are
responsible to:
- Provide him
with accurate and complete information about your history
and symptoms.
- Take an
active role in making decisions about treatment choices.
- Express
concerns about your care or service to Dr. Edwards
